Access to Medical Records

Patients can view their medical records in two ways.

Online Access to Medical Records

With effect from 31/3/2016, patients are able to view their detailed, coded medical record online and free of charge. This record will include diagnoses, test results, immunisations, medication issues and details of appointments.

Access is obtained through the practice’s patient portal, known as SystmOnline, which many patients already use to request repeat medication and book appointments. If you are not already using SystmOnline, you will need to register as a new user at our Reception desk. Please bring photo identification such as a passport, or driving licence.

Once registered as a SystmOnline user, you will need to read our leaflet which will draw your attention to some important things that you will need to consider when viewing your records. You will also need to complete our application form and hand it in to Reception.

Registration as a SystmOnline user and submission of the application form can be completed in a single, brief visit to our Reception desk.

Access to Medical Records under Data Protection Act 1998

Patients can also apply for hard copies of part, or all, of their medical record under the Data Protection Act 1998.

If your record is large and there is a lot of work involved in photocopying it, you may be charged up to £50, although we always try to keep these costs to a minimum.

If you would like to use this service, please complete this application form (instead of the one above) and hand it in to Reception. You will be contacted when your copy of the records is ready and you will need to bring a form of photo identification and payment when you come to collect it.
